Description: Recent research has demonstrated that two types of programs have potential for reducing avoidable health care and nursing home use, with one type of program focusing on the caregiver and the other focusing on the care delivery team. In this program, you will learn about the updated dementia care practice recommendations, the benefits of early detection and diagnosis, care planning and safety planning. This program will instill communication strategies to use with patients in a clinic/hospital setting. In addition, attendees will learn about available tool kits and community resources for patients and their care partner.
